# Notice of Privacy Practices

Cost Plus TRT LLC

Effective Date: January 1, 2026

## Purpose

This notice explains how we use and share your health information. It also explains your rights under federal law (HIPAA).

## Your Protected Health Information (PHI)

-   PHI includes health records, treatment, diagnosis, test results, payment information.
-   PHI is created or received by our clinicians, staff, and business partners.

## How We Use and Share PHI

We may use PHI for:

-   **Treatment** — clinical care, prescriptions, follow-ups.
-   **Payment** — billing, claims, verification of benefits.
-   **Health care operations** — quality review, training, audits.

We may share PHI with:

-   Licensed providers in your care.
-   Third-party service partners who agree to protect PHI.
-   Law enforcement or regulators when required by law.

## Uses and Disclosures That Require Your Written Authorization

We will obtain your permission before sharing PHI for:

-   Marketing communications that involve payment to us.
-   Sale of PHI.
-   Uses not described in this notice.

## Your Rights

You have rights with respect to your PHI:

-   **Access** — request a copy of your records.
-   **Amend** — request changes to your records.
-   **Restriction** — ask for limits on uses or disclosures.
-   **Accounting** — request a record of disclosures.
-   **Confidential contact** — ask for alternative contact methods or locations.
-   **Paper copy** — obtain a paper copy of this notice on request.

## How We Protect PHI

We maintain administrative, technical, and physical safeguards to protect your health information. We limit staff access to PHI to what is necessary for their role.

## Complaints

You may file a complaint if you believe your privacy rights were violated.

-   Submit a complaint to our Privacy Officer at the address below.
-   You may also file with the U.S. Department of Health and Human Services.

We will not retaliate against you for filing a complaint.
